Application for Letter of Good Conduct

The Campton Hills Police Department requires current and clear criminal history background checks prior the issuance of good conduct letters. Please complete the following form below and attach as images, two forms current and valid identification. Attached identification can include but are not limited to: Drivers License, U.S. Passport, Social Security Card, or State Birth Certificate. Records will be searched with the Campton Hills Police Records Management System. State and Federal criminal history checks must be made directly to those agencies via their approved forms. Completed good conduct letters can me mailed, emailed, or picked up from the Village Hall, after they are prepared by the Police Department. You will be contacted when it is ready, or if it cannot be provided. Applicants under 18 years of age must make their request in conjunction with their parent or legal guardian.

All sections of form must be completed in FULL in order to receive completed response by the Campton Hills Police Department. Responses may take 7-10 businesses days. Thank you for your patience.
